Perinatal Medizin

Care and support for normal and high-risk pregnancies

For both uncomplicated and high-risk pregnancies, a skilled team will care for you from the beginning of your pregnancy up until delivery. We will discuss any anomalies with you and make the optimal preparations for your child’s development and a successful birth.

Second opinions and support for obstetric questions and risks

If you have special concerns about your pregnancy, such as questions regarding illnesses like diabetes or coagulation disorders, or if you have experienced a difficult pregnancy or childbirth in the past, we would be happy to consult with you on these matters. We can answer your questions even if you are currently being treated by another physician.

Normal and high-risk pregnancies

Regardless of whether you would prefer a vaginal birth or a Caesarean section, we are open to your requests and opinions and will strive to fulfil them whenever possible. You can count on high-quality care based on Prof. Dr. Breymann’s extensive experience.

Obstetric surgeries and

Prof. Dr. Breymann has been delivering babies for more than twenty years. He is the Swiss representative for NESA (New European Surgical Academy) within the obstetrics division. NESA was founded by Prof. M. Stark, inventor of the “gentle Caesarean section” (the Misgav Ladach method), and Perinatal Zurich has been officially certified by Prof. Stark to use this surgical method.

Pregnancy ultrasounds, certified per SGUM (Swiss Medical Ultrasound Society) and DEGUM (German Medical Ultrasound Society) criteria

Prof. Dr. Breymann has been involved in developing pregnancy sonography for the last twenty years and has lived through all of its stages of development. With his experienced team, he manages the University Hospital of Zurich’s special ultrasound office hours. At the Perinatal Institute of Zurich, ultrasounds are performed and documented in accordance with the guidelines of the Swiss and German Medical Ultrasound Societies. All documented text and images are saved for you in your electronic pregnancy record.

3D / 4D ultrasound

Be delighted by 3D and 4D images of your baby. Depending on your baby’s position, 3D/4D images can be taken at any time during pregnancy and provide a fascinating look at your child’s development.

3D medical printing

Prof. Dr. Breymann is a member of the scientific advisory board of 3D Medical Printing AG, a pioneer in 3D medical printing. He worked with 3D Medical Printing to develop the technology for 3D imaging of your child.

First trimester (SGUM-certified) and organ ultrasounds (DEGUM-/SGUM-certified)

Ultrasounds during the first trimester (around the 10th-12th week of pregnancy) and second trimester (around the 20th-22nd week of pregnancy) are especially important for ruling out genetic abnormalities or congenital defects in your child. During the first-trimester ultrasound, we measure the nuchal fold, a skin fold near the child’s neck. A thicker nuchal fold (>3 mm) can be an indication of foetal genetic abnormalities or heart defects. For this reason, very precise measurements of the nuchal fold are of utmost importance. The main focus of the second-trimester ultrasound is to thoroughly examine the baby’s organs (brain, heart, gastrointestinal tract, kidneys, and bladder), as well as determining the weight of the baby and placenta, and confirming proper blood flow. Consequently, this ultrasound is extremely important for ruling out organ or other defects.

Amniocentesis (amniotic fluid sampling) and chorionic villus sampling

Amniocentesis and chorionic villus sampling are techniques for attaining foetal cells in order to perform chromosomal (genetic) tests. Both techniques are considered the gold standard of prenatal genetic tests, since the acquired foetal cells produce the most reliable results. Atypical results for an “NIPT” test (foetal DNA test using the mother’s blood sample) must always be confirmed via amniocentesis. In our experienced hands, both techniques are extremely safe. Both techniques are performed regularly at the Perinatal Institute.

Genetic consultations and foetal genetic test consultations

Due to the large variety of prenatal genetic tests offered today, a preliminary expert consultation on the possibilities and limitations of these tests is very important.
This consultation is also stipulated by the Human Research Act and may only be conducted by trained gynaecologists or geneticists. At our institute, our physicians and the Institute of Medical Genetics of the University of Zurich will provide you with thorough, expert consultations regarding genetic tests.

Consultations about defects or issues arising for the child during pregnancy

Should a problem such as inadequate growth or a defect arise in your child during pregnancy, we will consult with you on the possible consequences for the child during pregnancy and after birth. You will profit from our network of specialists who will discuss your case with Prof. Dr. Breymann, if you wish, in order to provide the best possible individualized care. This network, or “perinatal board”, is comprised of geneticists, paediatric radiologists, neonatal experts, paediatric surgeons, and other specialists.

General gynaecological surgeries

As a gynaecological specialist with many years of experience in gynaecological surgery, Prof. Dr. Breymann will perform any necessary interventions such as laparoscopies (abdominal endoscopy) or hysteroscopies (uterine endoscopy) using minimally invasive methods.
These interventions can be especially important for diagnosing issues about conception or repeated miscarriages caused by uterine defects, and diagnosing these issues can significantly improve your chances of a successful pregnancy.

DEGUM-certified mammograms

Prof. Dr. Breymann completed the German Medical Ultrasound Society (DEGUM) course in mammography and is certified to perform this procedure. Mammograms are very important imaging examinations in women’s preventive care, since mammograms are used to screen for malignant growths in the breast tissue.

Anaemia and iron deficiency office hours

Prof. Dr. Breymann is a pioneer in research in the area of iron deficiency in women. His research in this area has significantly influenced guidelines for treating iron deficiency in women both in Switzerland and on an international scale. He has written more than fifty publications and two books on the subject of iron deficiency in women. We can help treat your iron deficiency.

Pre-conception and pre-pregnancy consultations

We can help you plan your conception and pregnancy. Preparing for a pregnancy is important, and the time of conception and early pregnancy can be planned and positively influenced using orthomolecular treatments (such as vitamins, nutrients, and minerals). We will create a profile of your individual needs and plan a course of vitamin supplements accordingly. Prof. Dr. Breymann, in cooperation with “Burgerstein” (Switzerland’s most well known vitamin manufacturer), has developed his own line of vitamin supplements called “Women’s Ortholine”.

Paediatric gynaecology

Paediatric and adolescent gynaecology is an important gynaecological sub-discipline and corresponding services should be offered primarily by female gynaecologists. When they reach puberty and begin to menstruate, young girls often have questions and problems, which can be discussed or examined by a gynaecologist.

Contraception consultations

It is important for a girl or young woman to use an appropriate method of contraception, and this issue should be broached and discussed before she becomes sexually active. A thorough consultation discussing the benefits and drawbacks of different contraceptive methods and a gynaecological exam should take place before beginning a course of contraception.

HPV and dysplasia office hours for patients with cervical changes

Human papillomavirus (HPV) can cause changes to the cervix, and in unfortunate cases, can lead to cervical cancer. In order to detect this, physical examinations for women include regular “pap smears”. Both minor changes to tissue (dysplasia) and more noticeable changes must be thoroughly examined and clarified. In our HPV/dysplasia office hours, we thoroughly examine any changes to the cervix using a special microscope (colposcope), and we can also take tissue samples from any areas revealed by the colposcopy to be abnormal. Thanks to her years of experience in colposcopy, Dr. Manz is able to provide you with expert advice and information.

HPV vaccination

In the last few years, it has become possible to vaccinate young girls who are not yet HPV carriers against the virus before they become sexually active. This vaccination is the first vaccination for a cancerous disease developed in the world. We recommend that you vaccinate your daughter. Please visit us to discuss this matter.

Conservative gynaecology

(Deutsch) In der Frauenheilkunde gibt es eine Vielzahl von Störung bzw. Erkrankungen, die konservativ, also nicht mit Hilfe einer Operation behandelt werden.
Dies betrifft z.B. hormonelle Störungen, die Behandlung bei eingeschränkter Fruchtbarkeit, die Wechseljahre, Infektionen, die Empfängnisverhütung. Ausserdem zählen die Vorsorgeuntersuchungen ebenso zur konservativen Gynäkologie.

Physical examinations, screenings

(Deutsch) Die Vorsorgeuntersuchung, auch Screening-untersuchung genannt, dient der Früherkennung und bestenfalls der Vermeidung von Krankheiten. In der Frauenheilkunde wird sowohl die Anamneseerhebung, das ärztliche Gespräch, als auch die gezielte körperlich/gynäkologische Untersuchung dazu eingesetzt, Risiken herauszufinden und gezielt Krankheiten in der Frühform zu entdecken. Ein gutes Beispiel dafür ist das regelmässige Durchführen des Krebsabstriches (s.o.)

Prenatal care

(Deutsch) Eine Schwangerschaft ist in den allermeisten Fällen kein krankhaftes Geschehen und bedarf auch keiner medizinischen Behandlung im eigentlichen Sinne. Trotzdem sind regelmässige Besuche beim Frauenarzt/Frauenärztin sinnvoll und empfohlen. Bei der sogenannten Schwangerenvorsorge möchten wir herausfinden, bei welchen Frauen bzw. Kindern Veränderungen vorliegen, die potentiell oder tatsächlich krankhaft sind. In der Schwangerschaft ist eine gewisse Anzahl von Untersuchungen vorgesehen, um den optimalen Verlauf einer Schwangerschaft zu begleiten und nur bei Notwendigkeit medizinisch einzugreifen. Nach der Geburt, am Ende der Wochenbettsperiode (nach 6 Wochen) schliesst sich noch eine Abschlusskontrolle an.

Ultrasound during pregnancy (SGUM-certified)

(Deutsch) In der Schwangerschaft nutzen wir seit vielen Jahren die Methode der Ultraschalluntersuchung, welche für die werdende Mutter und für das Kind ungefährlich ist. Zu den verschiedenen Zeitpunkten der Schwangerschaft können wir verschiedene Strukturen beim ungeborenen Kind untersuchen (z.B. am Ende des dritten Monats die Nackenfalte, bei ca. 20 Schwangerschaftswochen die verschiedenen Organsysteme wie Gehirn, Herz, Wirbelsäule etc. und gegen Endes der Schwangerschaft kindliches Wachstum, Beschaffenheit und Lage des Mutterkuchens, Fruchtwassermenge und kindliche Lage). Damit diese Untersuchungen standardisiert und auf höchstem Niveau durchgeführt werden, bedarf es einer speziellen Ausbildung und Praxiserfahrung. Frau Dr. Manz ist für Ultraschall in der Schwangerschaft von der Schweizer Gesellschaft für Ultraschall in der Medizin (SGUM) zertifiziert.

Treating inflammation

(Deutsch) Im Bereich des äusseren aber auch des inneren Genitale kommt es manchmal zu Entzündungen, welche häufig ungefährlich aber meist sehr störend sind. Diese können oft einfach behandelt werden, indem man das richtige Medikament einsetzt und somit die Entzündung gezielt behandelt (z.B. juckende Pilzerkrankung der Scheide). Andere Infektionen können gefährlich sein (z.B. Chlamydieninfektion), sie müssen behandelt werden und man muss nach der Behandlung eine Kontrolle durchführen, damit man relativ sicher sein kann, dass es zu keinen Spätschäden kommt (z.B. eingeschränkte Fruchtbarkeit nach Entzündung der Eileiter)

Hormone consultations

(Deutsch) Für die Gesundheit jeder Frau spielen Hormone eine wichtige Rolle. In den verschiedenen Lebensabschnitten haben sie verschiedene Funktionen, so z.B. Entwicklung zur Geschlechtsreife in der Pubertät u. Adoleszenz, Ermöglichung von Fortpflanzung und Kontrolle über Menstruationszyklus während der Geschlechtsreife etc. Beim Zusammenspiel der verschiedenen Sexualhormone kann es Störungen und krankhafte Zustände geben, welche festgestellt und behandelt werden können. Ob beim Uebergang in die Wechseljahre eine Hormontherapie nötig ist, muss sorgfältig nach Erläuterung von Vor- und Nachteilen entschieden werden. Wir beraten und betreuen Sie gerne.

Gynaecological ultrasound examination

(Deutsch) Eine schmerzlose und ungefährliche Untersuchung ist die gynäkologische Ultraschalluntersuchung. Diese Methode ermöglicht es uns auf einfach Art u. Weise viel Information über die inneren Organe, Gebärmutter, Eierstöcke, Eileiter und Blase zu erhalten. Die Untersuchung kann zu jedem Zeitpunkt und ohne Vorbereitung durchgeführt werden.

(Deutsch) Basisabklärung bei Blasenbeschwerden u. Inkontinenz

(Deutsch) Ein häufiger Grund für den Besuch bei der Frauenärztin sind Blasenprobleme wie z.B. Blasenschwäche mit Urinverlust, häufiger Harndrang („Reizblase“) oder Schmerzen beim Wasserlösen. Mit Hilfe eines detaillierten Gesprächs und Erhebung der Krankengeschichte und einer einfachen Untersuchung kann man häufig schon die Ursache für die störenden Beschwerden herausfinden und oft mit einer Behandlung beginnen. Gelegentlich bedarf es einer detaillierteren Abklärung (urodynamische Untersuchung), für die wir sie gerne an einen Spezialisten/ eine Spezialistin weiter verweisen können.

(Deutsch) Beratung bei unerwünschter Schwangerschaft

(Deutsch) Nicht immer bedeutet eine Schwangerschaft uneingeschränkte Vorfreude und wird als willkommenes Ereignis angenommen. Gesundheitliche oder andere Probleme können dazu führen, dass Frauen eine Schwangerschaft in Frage stellen oder sich bewusst dagegen entscheiden. In solchen Fällen ist eine eingehende Beratung nötig, die Lösungsstrategien und/oder mögliche Alternativen aufzeigt und eine tragfähige Entscheidung ermöglicht. Diese Beratung erfolgt in einer ruhigen Atmosphäre in unserer Praxis. Sollte sich eine Frau gegen das Austragen einer Schwangerschaft entscheiden, so wird sie bei den weiteren Behandlungsschritten von Frau Dr. Manz begleitet.
